**Job Description**
This Ph.D. project aims to develop and optimize a Paediatric Post-Sepsis Model of Care to improve outcomes for children and their families after sepsis. Despite the increasing recognition of long-term sequelae from paediatric sepsis, current follow-up care is fragmented. The program seeks to create a comprehensive, consumer-informed, and scalable model. The Ph.D. research should complement existing grant work and may explore supplementary areas such as multimorbidity, health literacy, digital tools for follow-up, or nurse-led models of care. The successful candidate will join a collaborative environment, engaging with clinicians, consumer advocates, policymakers, and health service researchers.

• Doctor of Philosophy
• Background as a health professional or consumer advocate in health conditions
